Stipend / Registration Information
IMPORTANT NOTICE
If you apply for a stipend but want to purchase a third sleeping room night yourself, please do not make a room reservation with the hotel for that one night. Please wait to hear back from the Stipends Committee. Also, please include that extra night on your arrival and departure dates on the stipend application.
Please carefully read the instructions before filling out the forms. If you want to register for the conference only and do not plan to apply for a stipend, please use the Registration form. If you plan on applying for a stipend, please use the Stipend Application/Registration form.
If you need accommodations due to disability or respite, please complete the Accommodations form and submit with your Registration or Stipend form.
Registration Deadlines:
Friday, March 14, 2008 - Early Bird Discount Registration and Stipend Deadline
Monday, May 5, 2008 - Hotel Accommodations Deadline for conference $99.00 rate + tax
Monday, May 12, 2008- Regular Registration Deadline
Please submit one (1) form per person attending. PERSONAL ASSISTANTS MUST REGISTER SEPARATELY, but may apply for stipends.
NO REFUNDS WILL BE PERMITTED AFTER MONDAY, MAY 12, 2008
Remember to make your hotel reservation at the Nashville Airport Marriott before May 5, 2008 to receive the $99.00 +tax MegaConference rate by calling 1-800-228-9290 or 615-889-9300. (Unless you apply for a stipend. If you apply for a stipend your hotel room reservation will be made for you.)
Registration Instructions:
1. Register for the Conference! - (required) Please PRINT or TYPE CLEARLY. Include COMPLETE information. If you are NOT registering for the entire conference, indicate on the registration form which days you plan to attend.
• Breakfast and Lunch and Pirates Theme Night ARE INCLUDED in your registration fees for Wednesday and Thursday.
• The Awards Banquet and Dance is a special event and IS NOT INCLUDED in the Full Conference or Friday Registration.
• We hope you will choose to purchase a ticket to attend the Awards Banquet and Dance on Friday Evening.
**You MUST indicate if you plan to attend the Pirates Theme Night and/or Awards Banquet and Dance for planning purposes.
**If you register the day of the conference, NO MEALS ARE INCLUDED.
If you choose not to attend the events Thursday and Friday evenings, you will be ON YOUR OWN. Please plan accordingly.
2. Request a Stipend- (optional) Use the Stipend Application/Registration form. **Note: Stipends will be granted only to cover the registration fee and two nights’ stay at the hotel. Additional costs will be the responsibility of the person attending.
3. Accommodations/Respite-(optional) If you are in need of any special accommodations (ex. special dietary meals, conference materials in alternative formats, etc) or respite during the conference, please fill out the Accommodations form and return it with your Registration form and/or Stipend application.
This conference is FRAGRANCE FREE! Please refrain from using perfumes, colognes, or scented personal products in consideration of people with chemical sensitivities.
